Achieve Independence OT is committed to handling and protecting your privacy and personal information in accordance with the Australian Privacy Act (1988) and the NDIS Commission.

Collection of your information, Use and Disclosure

Achieve Independence OT acknowledges an individual’s right to privacy while recognising that personal information is required to be collected, maintained and administered in order to provide a safe working environment and a high standard of quality service.

We may collect personal information such as name, email address, contact number and other information you wish to disclose when completing an online contact form / referral form. This information collected will not be disclosed externally by Achieve independence OT.

The information collected is used to initiate a referral, to provide high quality service in a safe environment whilst meeting duty of care obligations. We will also use your personal details, phone or email, to provide you with further information, which you may have requested from Achieve Independence OT.
