Functional Assessments

Every person is individual and has their own unique needs and ways of doing things, so we spend time to get to know you, your home environment and complete a full Functional Assessment. We then outline your Occupational Therapy needs in a personalised report. These reports can be used to support you with plan reviews, Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA) assessments and with home modification and assistive technology applications.

Home Modifications

We understand the importance of your home, and want to strive to make your home environment set up to support your personal  needs, to maintain your independence and safety.
We conduct a home assessment to determine your functional need within your home environment, and how this may be best achieved. Home modifications can range from a simple grab rail to an accessible bathroom or ramp access.

Assistive Technology

We are here to help you find Assistive Technology that will allow you to be more independent and safe in your daily life. This equipment can range from simple shower chairs, to wheelchairs with complex seating. We work with suppliers so that you can trial the equipment in your home to ensure we get what is right for you.

Assistive technology can include;

  • Household equipment such as equipment used for cooking, showering or toileting
  • Mobility equipment including walkers, manual or power wheelchairs, scooters and pressure cushions
  • Transfer & manual handling equipment such as hoists, height adjustable beds, suitable seating and pressure care mattresses
  • Communication or Low vision Equipment

We discuss with you, assess your needs and assist with arranging trials of equipment in your home or the community and once provided we train in the use of assistive technology. We provide training for carers or family on how to use assistive devices or in manual handling.
